- - Date of payment
- - Amount paid (e.g. copies of cancelled check or credit card receipt)
- IF APPLICABLE - Copy of maintenance repair receipts (i.e. oil & filter change)
- IF APPLICABLE - Copy of bill of sale/sales contract
